I've been given this case to solve in an attempt to understand the topic of "patient clinical data analysis" to develop my competency in reading and comprehending clinical data including history, clinical findings, investigations and come up with a diagnosis and treatment plan. 

Case history :

34 year old female came to the casualty with the complaints of

Swelling in both the legs since 7 days

Abdominal distension since 7 days

History of present illness :

She was apparently asymptomatic 1 week ago then she developed swelling in both the legs suddenly upto the knee .It was sudden in onset, non progressive, pitting type of edema

She developed abdominal distension 6 days ago at the same time as swelling of legs which worsened for the past 2 days before admission 

She is having constipation since 3 days

She has decreased urine output for the past 1 day

She had burning micturiturition ,hematuria, frothy urine

H/o vomitings 4-5 episodes since 1 day which was non-projectile,non-bilious

History of bilateral lumbar pain which was non-radiating

Past history :
